Register for a lifestyle programme

Would you like to take part in a lifestyle programme? You can register via our appointment scheduler Simply register. We will then contact you to discuss your registration.

You can register for:

• a GLI programme, such as CooL, BeweegKuur or SLIMMER;
• it Stop Smoking Programme;
• a personalised lifestyle programme or a duo lifestyle programme;
• a coaching session.

Call us on 088-1180523. We are available from Monday to Friday from 08.00 to 17.00.

Reference required

To take part in a GLI programme, such as CooL, BeweegKuur or SLIMMER, you will need a referral from your GP or practice nurse. You can register with us in advance, but you cannot start until we have received the referral.
