Questionnaires
We use questionnaires. These help us to get a clear picture of your symptoms, health and circumstances before your treatment. The questionnaires also help us to find out how you are getting on, both during and after your treatment. This enables us to tailor your care more effectively to your needs. And it leaves more time during your appointment.
From time to time, we may ask you to complete a questionnaire. Below you can see which questionnaires these might be. Completing the questionnaires is voluntary and takes just a few minutes.
- A registration questionnaire after you have registered: Once you have registered, you will receive a questionnaire. In this questionnaire, we ask questions about your situation and the reason for your registration. We use your answers to assess whether your request for help is suited to the care services offered by our practice. We also look at which member of our practice is best placed to help you. Once we have reviewed your questionnaire, we will arrange an appointment.
- A general health questionnaire to complete before your first appointment: In this questionnaire, we take a broad look at your health and daily life. The questions cover, for example, how you feel, how active you are and what matters to you. This helps us to gain a comprehensive picture of your health.
- Once your treatment is complete: a questionnaire about how you are getting on now In this questionnaire, we ask how you are feeling and to what extent your symptoms still limit you in your daily life. This will give us a good idea of the outcome of your treatment.
Rondom Foundation
For every completed questionnaire, we donate a sum to our Rondom Foundation. Through this foundation, we help people who need healthcare but are unable to pay for it (in full). Scientific research
We can also use the results of completed questionnaires for scientific research. This enables us to further improve our psychological care. It helps us to underpin our care with scientific evidence.
