Interprofessional collaboration
At Rondom, we believe that good care and health are achieved when professionals from different disciplines combine their knowledge and expertise. Interprofessional collaboration means that professionals do not simply work alongside one another, but set goals together, make decisions and coordinate their care.
Within this field of research, collaboration itself is central. We are investigating how (healthcare) professionals both within and outside Rondom can collaborate effectively on care, health and prevention. In doing so, we focus, amongst other things, on communication and the exchange of information, the division of tasks and roles, joint decision-making, referral and feedback, coordination and digital support. We also investigate the organisational and human conditions required to implement new forms of collaboration sustainably in practice. By conducting research and applying the results in daily practice, we are building a form of collaboration that is person-centred, accessible and future-proof.
Collaboration between professionals centred on the individual
Health issues are often influenced by a combination of physical, mental, social, lifestyle and work-related factors. That is why we are investigating how professionals can work together to arrive at a broad and shared understanding of what is going on and what a person needs. In doing so, we look not only at symptoms and conditions, but also at functioning, resilience, wellbeing, participation, lifestyle and personal goals. This broad perspective is in line with the concept of positive health and helps to ensure that care and support are better tailored to people’s everyday lives.
Interprofessional collaboration can also help to identify health risks at an earlier stage and provide appropriate support in good time. That is why we are also investigating how professionals can share and combine signals from different disciplines. In this way, preventative action can be taken where possible, and it can be prevented that symptoms worsen or that people end up in reactive care unnecessarily.
